This course is designed as an overview of Diabetes and offers an introduction to the disease processes of Diabetes. It is suitable for any HCA / HCP supporting LTC reviews and capturing data for QoF. The purpose of this course is to provide a safe foundation of knowledge and a competency base.
The programme will be interactive and informative covering areas such as “What is Diabetes” and using games and group work to understand the importance of monitoring and providing essential information to support patient’s acceptance of diabetes.
HCAs will have information to guide the clinical review and support patients in the care planning and management of the disease.

- What is diabetes?
- Understanding Type 1 and Type 2 presentations
- Managing diabetes – both for the patient and for the team.
- Lifestyle risk factors and health promotion interventions
- Annual review- the importance of assessment and monitoring
Learning Outcomes
- To increase awareness of long-term conditions
- To recognise the differences and similarities between type 1 and type 2 diabetes
- To appreciate the importance of disease management: sign posting to the registered clinician for further assessment
- To have the skills to support the patient in change management
- To be able to accurately enter data to the records for QoF completion
